Our leave is administered through Sedgwick, and when I spoke to a Sedgwick rep yesterday they said I'd need paperwork from the hospital/from a pediatrician identifying me as her dad. This is more or less what they told Sam four years ago and she was able to take baby bonding leave with Koios.
