The FAA's Kelvin Coleman has stated that SpaceX will need to carry out a mishap investigation for the 3rd full stack Starship launch that occurred on March 14th

Each answer resolves NO if by that date there are no reports of the mishap investigation having been closed by the FAA

Once there are reports that the mishap investigation has been closed, all remaining answers resolve YES

If for some reason a mishap report does not end up being required, all remaining answers resolve N/A (previously resolved answers will not be reresolved)


US Eastern Time is used for consistency with the FAA's operations

Perhaps raises the issue of if we will hear about when the mishap investigation is actually closed? If there isn't much consequence to it, will it be seen as news worth reporting? Is there a way of looking up status?

@ChristopherRandles I'd say it's still likely it will be reported on when it happens but yeah that is a good thought. I'm just about to go to bed but will try to have a think/look in the morning

and this notified to SpaceX on 24 May 2024.

So presumably SpaceX have been able to start application for licence since then but yet granted.

I am tending to assume that investigation reached a point where they just needed more data from a future flight so mishap investigation not likely to be closed until some time after next flight. However that is just speculation and completely different reasons might be possible or more likely.
