Their name can be Samuel, Samantha, etc. as long as it's widely considered appropriate to call them "Sam".
The CEO can resign, be dismissed, get demoted, have the company go bankrupt, get reorged into a holding company... any event that causes them to no longer be considered "CEO of [company]".
