If true, this is another narrowly avoided nuclear war incident similar to those involving Stanislav Petrov and Vasily Arkhipov. (And it would be the only such case where the USA was responsible for the order rather than Russia.)

But this account of things is contested, and the Bulletin of Atomic Scentists appears to have quietly deleted their original article on the subject.

Resolves once enough information is declassified or otherwise comes to light for us to know with high confidence whether the described incident occurred.
