Will my next endoscopy still show up LA-D esophagitis?

NO

I have been first diagnosed with LA-C esophagitis last year with a 3 cm hiatal hernia. First round of treatment was PPIs with almost no changes to diet and lifestyle - result was esophagitis turned into LA-D. Second round of treatment was a change to the PPI and a few diet/lifestyle changes - result was still LA-D. I continued eating lots of high fat/friet food, including night meals little time before bed, loads of tea including mate which contains caffeine, and water minutes before sleeping.

Now I changed doctors, going to two of them and gathering different opinions, researching a lot more, going all-in in the GERD diet, I am consulting a specialized nutritionist, went through manometry and ph-metry test yesterday. Now taking Nexium and Motilium, raised my bed by about 15 cm to avoid night reflux. I am under a good health plan in São Paulo, going to the best hospitals in the country for the tests. I have been consulting GIs under the health plan, but I may try a fourth opinion outside it. I may follow advice in this market.

I have little symptoms, which is in one hand good, but which combined with the hiatal hernia is the reason why I suspect this is going to be difficult to treat by "knowing the triggers". I am a 33 year old male, not obese, non-smoker. The plan is to get my next endoscopy in about 4 months. Goal is to avoid Barrett and/or surgery.

  • Update 2024-18-12 (PST) (AI summary of creator comment): - The next endoscopy will be performed in early February while taking PPIs

    • The endoscopy's purpose is to evaluate treatment effectiveness while on PPIs, not to assess ability to discontinue PPI use

Most recent news:

  • Taking Nexium once a day, took Motilium (Domperidone) for a while, then stopped

  • Next endoscopy is going to happen in early February, and will happen while taking PPIs. Doctors think I'll have to be on PPIs for life (unless I do surgery), so the goal of this endoscopy is to ensure treatment is working, not to know if I can drop PPIs

  • I did a barium swallow test which showed up reflux until the middle third of the esophagus, but did not detect the hiatal hernia. Doctor explained it can miss if the hernia is not too big. Sometimes this test reveals the hernia is bigger than estimated through the endoscopy

  • Still doing the GERD diet, and read the "Acid Watcher Diet" book

  • Symptoms now are coughing about 5-10 times per day, no heartburn

So sorry to hear that. @GustavoMafra any news?
