The defect has to be a root cause, such as bad mechanical design leading to loss of hull integrity, it is not sufficient that the vessel was lacking safety fail-safes like a autonomous transponder.
Resolves when the answer is clear, likely what an official report is released at the latest.
I'm not sure I resolved this correctly TBH
It seems the half-baked monitoring system actually detected the damage on a previous dive, somehow working as intended, and they just ignored the data: https://youtu.be/FAAQVntpk00
Based on the debris it's clear the front fell off: https://youtu.be/CxBtZmyPzVA
As is well known the standards for maritime vessels are very strict on the fact the front is not supposed to fall off, thus best practices were not followed, resolves yes.
(references aside it seems pretty clear their brilliant plan to just slap some epoxy between very dissimilar materials ended predictably)
Based on we know now it was probably either faulty design of the viewport leading it to not having sufficient strength for the depth or repetitive stress without proper derating leading to failure of the carbon fiber hull. First is a clear YES but second feels more like maintenance issue, assuming the hull initially met the requirements? Any thoughts?
